Leadership & Care Coordination
The Administrator directs the agency’s daily administrative and financial operations, supports compliance with applicable health care requirements, and coordinates staff, resources, policies, reporting, and performance-improvement activities. This role works with agency leadership and clinical personnel to help ensure services are delivered responsibly, effectively, and under appropriate professional supervision.
The Director of Nursing supervises home health services and clinical personnel to help ensure care is safe, effective, and consistent with physician orders and each patient’s plan of care. This role supports clinical policies, staffing, orientation and education, interdisciplinary coordination, quality assessment and performance improvement, and communication with physicians, patients, families, and care professionals.
The Care Coordinator serves as a central point of communication, helping organize services, schedules, referrals, and changing care needs. By keeping patients, families, physicians, and care professionals informed and aligned, this role supports continuity and a more coordinated experience at home.
Nursing & Personal Care
Registered Nurses coordinate personalized nursing care, closely monitor each patient’s condition, and keep families, physicians, and caregivers informed. With a focus on safety, recovery, and education, they help every patient feel supported and confident at home.
Licensed Practical Nurses provide attentive, hands-on nursing care under the guidance of a Registered Nurse, including prescribed treatments, medications, monitoring, and documentation. They bring dependable care and reassuring communication to every visit.
Home Health Aides and CNAs offer compassionate assistance with personal care, meals, mobility, and everyday routines. They help maintain a safe and comfortable home while supporting each patient’s dignity, well-being, and independence.
Therapy & Rehabilitation
Physical Therapists develop personalized therapy plans to improve strength, balance, mobility, and range of motion. Through safe exercises, equipment guidance, and family education, they help patients move with greater comfort and confidence.
Physical Therapist Assistants carry out the Physical Therapist’s plan through guided exercises, walking practice, and functional training. They encourage patients, monitor progress, and help build greater strength, mobility, and independence.
Occupational Therapists help patients regain independence in everyday activities such as dressing, bathing, preparing meals, and safely navigating the home. They use practical exercises and adaptive strategies to make daily routines safer and easier.
Occupational Therapy Assistants help patients practice self-care skills, therapeutic activities, and adaptive techniques under the supervision of an Occupational Therapist. Through patient guidance and encouragement, they reinforce progress and make everyday routines more manageable.
